The aim of case management is to reintegrate people back into everyday working life after a long illness or accident.

Who is case management suitable for?
For people who have had to retire due to illness or an accident, or whose job is in danger and need support and coordination.
Action:
Registration of the person (private insurance, companies, pension funds, IV offices, social services)
Clarify the order with the client
Determining where you stand with the person (dealing with health challenges and developing coping strategies)
Joint definition of reintegration goals and determination of individually adapted steps
Clarification of the various stakeholder groups
Coordination of the helper network (insurance companies, therapists, doctors, employers, families, clinics, etc.)
Regular coaching of the person in dealing with stress, excessive demands and problems
Active individual support for employers
Regular feedback on the progress to the client
Continuous review of target achievement by means of queries, round-table and site discussions
Final report and any recommendations for action
